Replacement for single dextrose exchange as part of continuous ambulatory peritoneal dialysis (CAPD) or automated peritoneal dialysis (APD) regimen for the treatment of chronic renal failure particularly for some categories of patients who have lost ultrafiltration on dextrose soln because it can extend time on CAPD therapy in such patient.
Dosage/Direction for Use
Vol to be instilled should be given over a period of approx 10-20 min. Adult Instilled vol should not exceed 2 L. If this causes abdominal tension, 1.5 L vol should be used. Recommended dwell time: Between 6 & 12 hr in CAPD & 14-16 hr in APD.
Contraindications
Hypersensitivity to starch-based polymers eg, corn starch &/or icodextrin, maltose or isomaltose intolerance, glycogen storage disease, preexisting severe lactic acidosis, uncorrectable mechanical defects that prevent effective PD or increase the risk of infection, documented loss of peritoneal function or extensive adhesion that compromise peritoneal function.
Special Precautions
For IP administration only. Not for IV administration. Ensure completely clean during change procedure to reduce the possibility of infection. Carefully monitor to avoid over- & underhydration; overinfusion. Blood glucose levels should be regularly monitored in diabetics; adjust insulin dosage or other treatment for hyperglycemia prior to initiation of treatment w/ extraneal. Decrease in serum Na & Cl have been observed. Blood glucose measurement should be done w/ glucose-specific method to prevent maltose interference. Childn.
